How to get a CD or DVD after your examination
If a health professional asks you for a CD or DVD, go to the reception desk at Medical Imaging between 8:00 a.m. and 4:00 p.m. and ask for the CD or DVD of the desired examination. You will be asked to sign an Authorization to Release Information Contained in the Medical Record form and your CD or DVD will be given to you within an hour or so. You can also call the Imaging Service of the facility where you underwent your examination to have them prepare your CD or DVD. We will tell you when you can come and pick it up. You will have to sign an authorization form when you come to get your CD or DVD at the Medical Imaging reception desk. Please note that it is not possible to send anything to you by mail.
To have someone else pick up a CD or DVD of examination images, they must have with them a power of attorney signed by the person requesting the CD or DVD if the request is being made for someone aged 14 or over.
Note that there is a charge of $20 for any requests made for personal, legal or insurance needs. This procedure falls under the Regulation respecting fees for the transcription, reproduction or transmission of documents or personal information of the Act respecting Access to documents held by public bodies and the Protection of personal information.
